The cost is $36 for the full AMBA panel of 30 tests. Individuals taking the tests are required to fast, not eating any food or beverages, for 13 hours prior to their scheduled appointment time for the tests. Also available at additional cost are five specialized tests. AMBA requires that a separate physician order must be presented for any of the optional tests, which include Prostate Specific Antigen (PSA) test code 5363X for $25.70; Thyroid TSH test code 30163E for $12.00; Vitamin D-25 test code 17306X for $25.00; Hemoglobin A1C test code 496X for $12.00; and Insure Colon Rectal Kit test code 1129X for $20.00. The screenings are not covered by Medicare and most other insurance companies. Payment for the AMBA tests are to be made only with a check or money order made payable to AMBA Wellness Programs. An additional separate fee of $5, payable in cash or check, will be charged by Rotary to cover costs of advertising and light refreshments for individuals having blood screening tests.
